Pride Health is hiring a Case Manager to support our client’s medical facility in Jordon, UT . This is a 13-week assignment (with the Possibility of extension or conversion) and a great opportunity to start working with a top-tier healthcare organization!

Job Title: RN - Case Management

Location : West Jordan, Utah 84088

Schedule: Monday–Friday, 8:00 AM – 4:30 PM, 40 hours/week

Contract Length: 13 weeks with possible extension.

Job Summary:

The RN Case Manager reviews, assesses, and coordinates patient care plans to ensure appropriate utilization of resources and smooth transitions of care. Working collaboratively with interdisciplinary teams, the Case Manager facilitates discharge planning, documents clinical activities, and monitors treatment goals in compliance with regulatory guidelines. The role requires proactive management of length of stay and utilization review to optimize patient outcomes and efficiency.

Key Responsibilities:

• Conduct comprehensive assessments of patients’ physical, psychosocial, cultural, and spiritual needs.
• Utilize case management software to document and track patient care and referrals.
• Facilitate discharge planning and coordinate with patients, families, and healthcare teams.
• Monitor utilization and ensure compliance with medical necessity criteria.
• Participate in interdisciplinary rounds and performance improvement initiatives.
• Communicate treatment goals and variances to the care team.
• Collaborate with social workers, utilization reviewers, and third-party payers.

Qualifications:

• Associate Degree in Nursing required; Bachelor’s preferred.
• Current RN license (Utah or participating NLC state).
• American Heart Association Basic Life Support (BLS) certification.
• Minimum 1 year of nursing or case management experience.
• Knowledge of regulatory requirements and accreditation standards preferred.
