Job Summary

The Community Health Educator (CHE) will be an individual with an interest in community health and positive youth development. This position will work with teens, parents and other adults, providing school and community-based health education, promotion, and outreach with a focus on substance use prevention, problem gambling prevention, and mental health awareness. This role is part of the Youth Initiatives program within the Department of Community Health. The CHE will work with program staff to implement youth-centered programming.

Duties Include

Provides health education and advocacy for teens, families, and other community members

Designs, develops, implements and leads health promotion, education and programming for teens and young adults.

Recruits teens through community partnerships, social media, and other outreach methods

Links clients with necessary school, health and community resources

Participates in program planning, implementation and evaluation activities

Conducts social determinants of health screenings, referrals, and ensures connection to services in a culturally and linguistically appropriate way.

Does follow up with teens and others as appropriate to ensure connection to services and records interactions in EPIC, spreadsheets (i.e. Sheets) or other databases as needed.

Qualifications

Education/Training: Bachelors degree preferred, though relevant experience is highly valued and preferably with experience in public health, social work, healthcare, education, and/or human services; community health worker training and/or certification desirable.

1 years experience in community education, outreach, or health/human services preferred.

Minimum 1 year of direct-youth programming experience in a youth-serving, community health or clinical environment.

1 year of experience working within and/or navigating the healthcare environment preferred.

Understanding of and commitment to health equity and racial justice principles.

Ability to travel to and among multiple sites within a workday located within the service area.

Oral and written fluency in English. Second language desired, Spanish, Portuguese and/or Haitian Creole preferred.
